You wake up the morning after a hard workout and something hurts. Maybe it’s your hamstring. Maybe it’s your calf. It aches when you walk, protests when you climb stairs, and now you’re lying in bed at 6 a.m. running through the same loop: Is this just soreness, or did I actually hurt myself?

This is one of the most common and most anxiety-producing questions in fitness — and it’s surprisingly difficult to answer if you don’t know what to look for. Push through an injury and you can turn a minor problem into a major one. Baby a case of normal muscle soreness and you lose training days you didn’t need to lose.

The good news is that your body is actually pretty good at sending the right signals. You just need to know how to read them. The old explanation — that it’s lactic acid buildup — was largely debunked decades ago. The more current and widely accepted view is that DOMS is linked to microscopic damage to muscle fibers, especially after eccentric movements where muscles lengthen under load, which triggers an inflammatory response as your body works to repair and rebuild the tissue. What Does a Real Muscle Injury Feel Like?

A true muscle injury — most commonly a strain, which involves partial or complete tearing of muscle fibers — is a different animal. A true muscle strain involves actual tearing of muscle fibers, often from overstretching or forceful contraction, and unlike DOMS, it tends to be immediate — you may feel a pop or snap, followed by pain, swelling, and limited ability to use the muscle.

1. When Did the Pain Start?

DOMS: Pain arrived after the workout — not during it. You felt fine at the time and noticed discomfort anywhere from a few hours to the next morning.

Injury: Pain arrived during the workout, often at a specific identifiable moment. You may have felt something go wrong in real time — a sharp sensation, a pop, a sudden onset of pain that stopped your movement.

Noting the time symptoms begin is one of the most reliable ways to differentiate between soreness and a potential injury. DOMS is almost always a delayed experience. True injury pain is usually immediate or close to it.

2. What Does the Pain Actually Feel Like?

DOMS: Dull, aching, diffuse. Like a deep muscular heaviness or tightness. It’s uncomfortable but not alarming — similar to the feeling of very stiff muscles after a long day of physical labor.

Injury: Sharp, stabbing, or burning. A more acute quality of pain that makes you catch your breath, wince, or instinctively stop what you’re doing. If you have localized pain that is sharp, limits your mobility, changes your gait, affects your range of motion, or is accompanied by significant weakness, it could be a sign of a serious injury that needs treatment.

3. Where Is the Pain — and Is It Symmetrical?

DOMS: Usually spread across a muscle group rather than pinpointed to one exact spot. It often affects both sides of the body roughly equally — both quads, both calves, both shoulders. Diffuse muscle pain that affects multiple limbs is a characteristic feature that helps clinically differentiate DOMS from a muscle tear or strain.

Injury: Typically localized to one specific area, often to the point where you can press your finger on the most painful spot. 4. How Does It Respond to Gentle Movement?

DOMS: Stiff and uncomfortable when you first start moving, but the discomfort eases as you warm up. A gentle walk, some light movement, or easy cycling often makes DOMS feel noticeably better within 10-15 minutes of low-intensity activity.

Injury: Pain that persists or worsens with movement, or that makes you instinctively guard and protect the area. If gentle activity makes it worse rather than better, or if you find yourself unconsciously changing your movement patterns to avoid loading the area, treat it more seriously.

DOMS is a dull, widespread ache, while an injury tends to be sharp and sudden — and the simplest test is how the discomfort behaves with gentle movement.

5. Is There Any Visible Swelling, Bruising, or Deformity?

DOMS: No visible changes to the affected area. The muscle might feel slightly firmer to the touch due to inflammation, but there’s no swelling you can see, no bruising, no visible change in the shape of the muscle or joint.

Injury: Signs of an acute injury include swelling, bruising, or significant limping. Visible bruising that appears after exercise almost always indicates tissue damage beyond normal soreness. Swelling, particularly localized swelling around a joint, is another injury signal. If the muscle looks or feels different in shape compared to the other side, that’s a more serious concern.

6. Can You Move the Joint Through Its Normal Range?

DOMS: You can complete a full range of motion through the affected area, even if it’s uncomfortable. Sore quads will still bend fully — it might not feel great, but you can do it. Sore shoulders can still reach overhead.

Injury: Movement is restricted — not just because of discomfort, but because the joint or muscle genuinely won’t go where it’s supposed to go. An injury might limit normal functions like walking, sitting, or picking up objects. 7. How Long Has It Been Going On?

DOMS: Follows a predictable arc. It shows up within 24 hours, peaks around 48-72 hours, then steadily fades. If soreness lasts far longer than a few days or keeps getting worse, treat it as a possible injury rather than ordinary DOMS.

Injury: Pain that doesn’t follow the expected DOMS arc — that shows no improvement after 5-7 days, that continues to worsen after the first 24-48 hours, or that fully resolves and then comes back when you try to train — is more consistent with an injury than normal soreness.

8. Does Pressing the Area Cause Sharp, Pinpoint Tenderness?

Gently press on the center of the sore area with your fingers.

DOMS: The whole general region is tender and uncomfortable when pressed — but there’s no single spot that causes a dramatically sharper or more intense pain response than the surrounding tissue.

Injury: A specific, localized point of extreme tenderness that’s clearly worse than the surrounding area when pressed. In muscle strains, there’s often a point of maximal tenderness that corresponds to the area of tissue damage. The Injury Red Flags: Stop and See Someone If You Have Any of These
